9. What forms of payment are accepted? We accept all major credit and debit cards for both annual and monthly payment options.

10. How do I schedule my appointments? Once you sign up, you can schedule your appointments through our website or by calling our clinic. We offer priority scheduling for plan members.

11. What if I need to reschedule my appointment? If you need to reschedule, please contact our clinic at least 48 hours in advance to avoid any cancellation fees.

WSD Wellness Plan Terms and Conditions

1. Introduction: These terms and conditions apply to the WSD Wellness Plan ("Plan") between you (the customer/patient) and WSD in our capacity as a health service provider. Please note that your visits to any WSD Practice(s) and the services we provide are subject to our standard terms and conditions.

2. Privacy and Use of Personal Information: All personal information is handled according to our Privacy Policy. Your use of the WSD website is governed by our Website Terms of Service.

3. Plan Overview

The WSD Wellness Plan is a 12-month subscription service providing specified treatments and discounts as outlined in clause 4 (“Benefits”) at participating WSD practices. The Plan automatically renews for another 12-month period unless you cancel or notify us of your decision not to renew before the current term ends. Detailed terms and conditions are explained in the sections below.

4. Benefits

4.1. During the 12-month term, the Plan offers the following benefits:

- One standard examination and X-rays

- Two 45-minute hygiene appointments

- 5% off additional treatments, excluding dental implants, orthodontic services, oral medicine services, sedation (all forms), and specialist care.

4.2. Please note:

- The Plan may not be suitable for all patients. Your dentist can advise on the best treatment plan if this Plan is not suitable for you.

- If advanced or additional hygiene care is needed, an extra fee will be charged. You will be informed of this cost before the treatment is completed.

- It is your responsibility to utilize the benefits before the end of the 12-month term.

5. Registration and Patient Portal Use

5.1. When purchasing the Plan, you must provide a valid email address ("contact details"). WSD uses this to give you access to the patient portal and to communicate updates about your plan, including renewal notifications and changes.

5.2. You must inform us in writing if your contact details change. Otherwise, we cannot contact you regarding the end of the 12-month term and plan renewal.

6. Participating Practices Only

6.1. The Plan is valid only at participating WSD practices ("Participating Practices").

6.2. The Plan cannot be used at any non-participating practices, including those specializing in oral surgery, endodontics, or orthodontics.

6.3. The Plan must be used at the Participating Practice where it was purchased. If purchased online, it is valid only at the selected practice.

6.4. If your chosen practice stops offering the WSD Wellness Plan, WSD will work with you to find a solution, though the final decision rests with WSD.

7. Term and Renewal

7.1. The Plan and its benefits are valid for 12 months from the signup date.

7.2. Upon registration, you commit to the full 12-month term.

7.3. If you cancel the Plan before the end of the term, no refund is given, and any outstanding payments as per clause 10 are still due.

7.4. The Plan automatically renews at the end of each 12-month term. To cancel, notify WSD by email at info@wsd.nz before the renewal date.

7.5. You will receive an email reminder about the Plan renewal approximately 28 days before the end of the term, and a confirmation email upon renewal.

8. Pricing

8.1. You will be informed of the total amount payable for the 12-month term before registration ("Plan Price").

8.2. WSD reserves the right to change the Plan Price at any time, with changes applying at renewal. Existing customers will be informed of any price changes in the renewal email.

8.3. Some patients may purchase the Plan retrospectively within 24 hours of an initial hygiene or exam and X-ray appointment, at the discretion of the Practice.

9. Payment

9.1. The Plan is a 12-month subscription paid by direct debit from your credit or debit card.

9.2. Payments can be set up on a monthly recurring basis or annually in advance at purchase and cannot be changed later.

9.3. The Plan cannot be paid using Q-card finance offers, Afterpay, or other financing methods unless specified.

9.4. You authorize WSD to deduct Plan payments automatically.

9.5. If payment fails, WSD will retry up to 3 times. If payment still fails, the WSD Practice Support team will contact you for alternative payment or to cancel the Plan according to clause 10.

9.6. WSD may deny or cancel the Plan if payment terms are not met, with no refund for cancelled plans and outstanding payments due as per clause 15.

9.7. Stripe New Zealand Limited ("Stripe") is the payment gateway used by WSD. You authorize WSD to share necessary personal information with Stripe.

10. Cancellation

10.1. You can cancel your Plan anytime by providing written notice to your Participating Practice.

10.2. If you cancel within 7 days of registering or renewing the Plan ("Cooling-off Period"), you will be charged for all Benefits used before cancellation at the full retail price of the Participating Practice.

10.3. If you cancel after the Cooling-off Period or if WSD cancels for non-payment, you will owe:

- The unpaid balance of the remaining Plan Price; or

- The full retail price of used Benefits, minus any Plan payments made.

10.4. WSD will inform you in writing of the amount owing upon cancellation, and you authorize WSD to charge this amount using your nominated payment method.

11. Limits on Use of 5% Discount

11.1. The 5% treatment discount cannot be combined with other offers or promotions, including Southern Cross discounts, corporate offers, dental exam & X-ray offers, new patient offers, or GoldCard discounts. You may choose which eligible discount to apply at payment.

11.2. The 10% discount is not applicable with government-funded programs like Ministry of Health Dental Benefits, WINZ payments, or ACC payments. You may choose which discount to apply at payment.

11.3. The discount cannot be used with Q-card finance offers or Afterpay. You may apply the Plan discount or use these payment methods for the standard treatment cost.

12. Data Retention and Deletion

12.1. WSD retains personal information as required for providing the Plan, renewing the Plan, pursuing legitimate business interests, making data-driven decisions, conducting audits, complying with legal obligations, resolving disputes, and enforcing agreements.

12.2. Information will be deleted upon request unless legally required to retain it. For queries, contact info@wsd.nz. More details are in our Privacy Policy.

13. Additional Notes

13.1. The Plan is non-transferable and cannot be redeemed for cash.

13.2. Plan benefits cannot be paused or deferred and do not roll over to subsequent Plans.

13.3. Patients are responsible for informing Participating Practices of any changes to contact details.

13.4. Appointment changes must comply with the Practice's cancellation policy. Failure to cancel without 24 hours’ notice may result in the treatment being considered redeemed.

13.5. If your provider is unavailable, another qualified provider will be assigned. WSD will attempt to notify you of any changes.

14. Excluded Treatments

Orthodontics may involve the use of (but is not limited to):

• Fixed braces

• Removable braces

• Clear aligner therapy (such as SureSmile)

• Functional appliances

• Space maintainers

• Orthodontic retainers

Dental Implants are often used to replace missing teeth. They can also support fixed or removable prostheses or dentures and will usually involve both a surgical (placement) and prosthetic (restoration) phase. Dental implant therapy may involve the use of (but is not limited to):

• Surgical placement and/or bone grafting

• Intravenous sedation

• Fixed implant supported crowns and/or bridges

• Implant supported dentures both fixed and removable
